Immunization Requirements for grades 6-8 from Illinois Department of Public Health (IDPH) and Illinois State Board of Education (ISBE):

  • Any child entering 6th grade shall show proof of receiving one dose of Tdap (tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last DTap, DT or Td does and 1 dose MCV4 (meningococcal).
  • Students entering grades 7-8 who have not already received Tdap are being encouraged to receive 1 Tdap dose regardless of the interval since the last DTap, DT or Td dose.

Whooping Cough, also known as pertussis, continues to occur in Illinois. In an effort to protect children and adolescents from the serious complications associated with this disease, all students entering grades 6 and 9 will be required to present proof of vaccination. Please make an appointment with your health care provider now to receive this important vaccination or to verify that your child has received this booster. 

Physical Forms & Immunizations for Early Childhood, Pre-K, K, 6th and 9th Grades

  • Child Health Examination forms and required immunizations MUST be submitted prior to the first day of school in order for students in K and 6th  grades to begin school.
  • Early Childhood (including Walk-in Speech students) & Pre-K students must submit required health records and immunizations prior to the start of services.
  • The physical exam must be completed within one calendar year prior to the start of school and must fulfill all Illinois State Board of Education and Illinois Department of Public Health requirements.
  • Students will not be allowed to begin school without a submitted physical examination form and required immunizations.
  • Students who are new to District 258 and IL schools must submit health records within 30 days of entering school.  Transferred health records will be reviewed by the school nurse for compliance with Illinois school health requirements.
  • ​​​Illinois State law now mandates that all K, 2 and 6 grade students present proof of a dental examination prior to May 15 of the school year.
  • A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to a dentist.

Vision Exam Requirements

  • Illinois State law now mandates that all incoming K and students new to Illinois Schools must present an eye exam report conducted by a physician who provides a complete eye exam or a licensed optometrist.
  • This report must be submitted to the school by October 15 of the school year and must be completed within one year prior to the required date.
  • A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to an ophthalmologist or optometrist.

Dental Exam Requirements

  • Illinois State law now mandates that all K, 2 and 6 grade students present proof of a dental examination prior to May 15 of the school year.
  • This exam information must be less than 18 months old.
  • A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to a dentist, or religious exemptions.
